关于阅读《旅客告知书》并填写《个人承诺书》的相关通告

尊敬的旅客：

根据首都严格进京联防联控机制工作要求，为全力配合做好北京市新冠肺炎输入性疫情防控工作，按照民航局要求，现将有关要求通告如下：

14天内入境且目的地为北京（含第一入境点已调整为其他城市）的旅客，在登机前均需阅读《旅客告知书》并签署《个人承诺书》。

请旅客朋友认真阅读并如实填写，感谢您的理解与配合。

 

旅客告知书

根据中华人民共和国海关总署关于《中华人民共和国出入境健康申明卡》最新版的要求，所有旅客如实申报相关信息，其中包括过去72小时内是否服用退烧药、感冒药。

同时中华人民共和国最高法院、最高检察院、公安部、司法部联合发布《依法惩治妨害新冠肺炎疫情防控违法犯罪的意见》中明确规定:对于已经确诊或疑似新冠肺炎病人，进入公共场所或者公共交通工具造成新冠肺炎病毒传播的，属于抗拒疫情防控措施的犯罪情形，将按照“以危险方法危害公共安全罪”定罪处罚。

对于新冠肺炎确诊者、疑似者或密切接触者和有发热、干咳、乏力、头痛等症状的旅客不应乘坐飞机出行;对于14天内入境的旅客，登机后应服从乘务员的安排，在机上指定区域或隔离区就座，不得随意走动;飞机抵京后需单独接受尿检、血检等检测。

北京市政府自2020年3月16日零时起，对所有境外进京人员，均转送至集中观察点进行14天的隔离观察，集中隔离观察期间，隔离人员费用需要自理。对于虚报信息，隐瞒病情，造成疫情传播的人员将依法依规追究责任，并纳入信用体系。

（友情提示:鉴于近期入境人员较多、疫情排查需要额外的时间，故入境、过站等待时间可能较长，请您提前做好相关准备并携带必要的食品、药品、儿童用品等生活必需品。）

个人承诺书

我已阅读《旅客告知书》，知悉《依法惩治妨害新冠肺炎疫情防控违法犯罪的意见》中相关规定和《中华人民共和国出入境健康申明卡》有关要求，申明过去72小时内未服用退烧药和感冒药。

本人承诺将如实申报健康状况，未被医疗机构认定为新冠肺炎或疑似病例，无发热(≥37.3℃)、干咳、乏力、头痛等症状，本人未使用退烧药或采取其他方式降温。如有不实申报，将按照中国法律法规及有关部门的规定承担法律责任。

Passenger Notification Letter

According to the requirement of the latest edition of The Entry and Exit Health Declaration Card of The People’s Republic of Chinaissued by The China Customs , all passengers shall truthfully report relevant information, including whether you have taken antipyretics or cold medicine within the past 72 hours.

People’s Republic of China, The Supreme Procuratorate, The Ministry of Public Security and The Ministry of Justice, it clearly states that patients who have been diagnosed or suspected of the Novel Coronavirus Pneumonia (NCP) enter public places or take public transport causes the spread of novel coronavirus committed crime resisting epidemic prevention and control measure and shall be convicted and punished according to “crime of endangering public safety in a dangerous way”.

Those who have been diagnosed with the Novel Coronavirus Pneumonia (NCP), suspected or close contacts, and passengers with fever, cough，obey the arrangement of the flight attendants after boarding, and be seated in the on-board designated or isolation zone and do not walk around at will. After arriving at Beijing, separate urine and blood tests are required.

Beijing Municipal Government announces that from 00:00 on March 16th 2020, all the persons that enter into Beijing from abroad shall be transferred to a centralized observation point for a 14 days isolation quarantine observation. During this period, all the expenses shall be borne by themselves. For those persons who submit false information, concealillness that causes the spread of novel coronavirus shall be held accountable according to laws and regulations and be incorporated into the credit system.

(Kindly reminder: in view of the large quantity of recent arrivals, extra time is required for epidemic investigation. Therefore, the waiting time for entry and transit may be much longer. Please get prepared in advance and bring necessary food, medicine, children’s products and other necessities.)

I have already read Passenger Notification Letter and understand the relevant regulations and requirements in have not been diagnosed or suspected of antipyretics or other methods to lower down my temperature.
